Pulp Therapy & Zirconia Crown for Childrens Milk Tooth
Some of the most important dental work we do is on our smallest patients. This case is about an 8-year-old who came to our Sithalapakkam clinic with pain and difficulty eating. Extensive decay and a fracture in a lower primary (milk) molar were to blame. Below is what we found during the clinical evaluation, how pulp therapy and a zirconia crown saved the tooth, and how the child was soon back to eating, smiling and playing normally.
Before and after treatment
The patient at a glance
A young patient arrived with a toothache that had been affecting eating for a while. Here is a summary of the case:
- Patient: an 8-year-old child
- Chief concern: pain and difficulty eating
- Diagnosis: extensive decay and fracture of a lower primary (milk) molar
- Treatment: pulp therapy to treat the affected tooth, followed by tooth restoration and a zirconia crown
- Outcome: comfortable, pain-free and eating normally again
Why a decayed milk tooth needs treatment, not extraction
It is a common question from parents - if the tooth will fall out anyway, why treat it? Because a milk tooth is not disposable. A primary molar holds the space for the permanent tooth developing underneath, guides neighbouring teeth into position, and is needed for chewing. When a milk tooth is lost early, the teeth around it drift into the gap, and the adult tooth may come through crowded or stuck. Treating the decay is often simpler, kinder and cheaper than correcting all of that later.
how children's pedodontic management protects milk teeth kids dentistry at Spark Dental Care
- Milk teeth hold open the space for the permanent teeth waiting underneath
- They guide neighbouring teeth into their correct positions
- Losing a molar early can mean crowding and orthodontic work later
- Infected milk teeth cause pain, swelling, and disturb eating and sleep
- Early treatment keeps the repair small, quick and child-friendly
What is pulp therapy for kids?
Pulp therapy is the child-friendly way to save a milk tooth when decay has reached the nerve inside it. The infected pulp is removed, the inner chambers are cleaned and sealed, and the pain is eliminated. In an adult tooth the same principle is called root canal treatment; in a milk tooth it does the same job while keeping the tooth in place so the permanent tooth underneath stays protected.
how root canal treatment works kids dentistry
- Removes the infected pulp that is causing the pain
- Cleans and seals the inner chambers of the milk tooth
- Relieves pain so the child can eat, sleep and play comfortably
- Saves the tooth instead of extracting it prematurely
- Protects the permanent tooth developing underneath
Why a zirconia crown after pulp therapy
After pulp therapy, the treated tooth is more fragile and needs protection - which is where the crown comes in. We restored this child's lower molar with a zirconia crown. Zirconia is a strong, metal-free material that shields the tooth while it stays in the mouth, and because it is tooth-coloured, it looks entirely natural. For a back milk tooth that has to handle heavy chewing until it is ready to be replaced, zirconia is an excellent choice.
restorative dentistry and crowns the different types of dental crown
- Protects the treated tooth from fracture during everyday chewing
- Metal-free, so nothing darkens the tooth or the gum over time
- Tooth-coloured and natural-looking, even on a visible tooth
- Strong enough to last until the milk tooth is ready to fall out
- Comfortable for a child to eat, chew and smile with
The result
After the clinical evaluation, pulp therapy removed the infection, the tooth was restored, and the zirconia crown sealed everything safely in place. The pain that had been disturbing this child's eating was gone, and the tooth was strong and natural-looking again. Within days, the child was comfortable, pain-free and able to eat normally - in short, back to being a kid again.
What this case teaches
Two lessons from this case apply to every family. First, a toothache in a child is never a small thing - decay that reaches the nerve of a milk tooth can ruin eating, sleeping and confidence, and it always gets worse when it is left. Second, the milk tooth does not need to be lost: with pulp therapy and a crown, it can be saved and made to last until the permanent tooth is ready. If your child complains of pain, avoids chewing on one side, or has a dark or broken milk tooth, a check-up is the fastest way to protect both the baby tooth and the adult tooth underneath.

Frequently asked questions
If a milk tooth will fall out anyway, why treat it?
A milk tooth does more than hold a gap. It keeps the space open for the permanent tooth underneath, guides neighbouring teeth into position, and is needed for chewing. If a milk molar is lost early, the teeth around it drift together and the adult tooth may come through crowded or blocked. Treating the decay with pulp therapy and a crown keeps the milk tooth healthy until it is ready to fall out on its own - which is usually the simplest and least expensive path.
Is pulp therapy painful for a child?
No. Pulp therapy is carried out under local anaesthetic, so the child feels little or no discomfort during the procedure. In this case the therapy actually removed the source of the pain, and the child was comfortable, pain-free and eating normally again soon after. We keep appointments short, explain everything in a child-friendly way, and stop whenever the child needs a break.
What is the difference between pulp therapy and root canal treatment?
The idea is the same - remove the infected pulp from inside the tooth, clean and seal the chambers, and save the tooth from extraction. Root canal treatment is the term used for adult teeth. Pulp therapy is the kid-specific way of doing this on a milk tooth, using techniques designed to preserve the developing permanent tooth underneath, which is why it is performed only on children.
Why use a zirconia crown instead of a metal or strip crown for a child?
Zirconia crowns are strong, metal-free and tooth-coloured, so they do not darken the tooth or the gum over time and they look natural when the child laughs. They are particularly well suited to back milk teeth that need protection during heavy chewing, which is why we chose one for this child's lower molar. The crown stays in place until the milk tooth is ready to be replaced by the permanent tooth.
Can a child eat normally after getting a zirconia crown?
Yes. The crown rebuilds the chewing surface of the tooth and is fixed firmly in place, so the child can eat normally right away. This child - who had been struggling with pain and difficulty eating - was soon comfortable and eating normally again, which is exactly what we expect after pulp therapy and a crown are completed.
